The Lead Physician provides direct clinical care to a panel of members and serves as a clinical leader within the center. This role guides clinicians in delivering value-based care and partners with the Medical Director to lead providers, nurses, behavioral health specialists, and medical assistants. Key Responsibilities •Deliver excellent primary care to a panel of members and lead your interdisciplinary care team. •Coach and mentor providers on best practices for primary, urgent, and community care; support population health huddles to meet quality and utilization goals. •Serve as the center’s clinical leader alongside the Medical Director, providing guidance to care teams and addressing clinical questions. •Participate in leadership meetings and represent AbsoluteCare to community organizations, hospital systems, and payer partners. Duties •Provide primary care and manage acute exacerbations of chronic conditions. •Accept urgent appointments post-hospital or ER discharge to prevent readmissions. •Collaborate with social work, behavioral health, nursing, pharmacy, and other team members to implement care plans. •Track and achieve quality metrics; practice evidence-based medicine within the PCMH model. •Develop transitional care plans and monitor patients across the continuum of care. •Use technology and EHR to optimize quality and efficiency. •Participate in multidisciplinary meetings and on-call rotations. •Develop clinical protocols to upskill staff. •Approximately 70–80% of time dedicated to direct clinical care. Minimum Qualifications •Active, unrestricted medical license in the state of employment. •5+ years of clinical and administrative experience leading interdisciplinary teams. •Experience in clinic, community-based, and inpatient settings. •Strong mentoring, communication, and problem-solving skills. •Independent, organized, and passionate about population health and care model innovation.
